The introduction of polyvinylpyrrolidone into the joint cavity, especially in large joints, 30-50 ml with an interval of 5-7 days up to 3-5 times, has not lost its significance. In its physicochemical properties it is close to synovial fluid and has an anti-inflammatory and normalizing effect on the permeability of the synovial membrane. Pharmacological correction of impaired proteoglycan metabolism in articular cartilage is indicated. Chondroprotective agents are prescribed 1-2 times a year; they stimulate restoration processes in the affected cartilage and increase the synthesis of proteoglycans by chondrocytes. Rumalon, arteparon and mucarton are used for these purposes. Rumalon is administered intramuscularly, 2 ml every other day, for a course of up to 25 injections. Arteparon is a complex of mucopolyoncharides with a high sulfate content. It is administered intramuscularly, 1 ml of a 5% solution 2 times a week for 8 weeks. Mucarton is close in composition to arteparone, use 1 ampoule daily for 10 days, then 1-2 times a week up to 25 injections per course.

    In recent years, a large place in the treatment of arthrosis disease has been given to the prescription of NSAIDs, which have anti-inflammatory, antipyretic and analgesic effects. Recommended doses for DOA:

    Diclofenac 1-3 tablets per day (75-150 mg),

    Ibuprofen 3-6 tablets per day (100-200 mg) per day,

    Piroxicam 1 tablet per day (20-40 mg).

    Nimesulide 1 powder 2 times a day (100-200 mg).

    As a rule, they are prescribed for 1-2 weeks until pain and effusion are relieved. The use of NSAIDs for diseases of the gastrointestinal tract (peptic ulcer, gastritis) is not indicated. NIMESIL, a new generation NSAID, has proven itself well, selectively blocking the activity of cyclooxygenase-2, an enzyme that plays a key role in the development of inflammation. The drug does not have a negative effect on the gastrointestinal mucosa.

    Based on NSAIDs, a large number of local products have been created in the form of ointments, gels, balms that have antipyretic, anti-inflammatory, and analgesic local effects (chondroxide, indomethacin, diclofenac, fastum-gel ointments).

    For persistent recurrent synovitis, intra-articular administration of corticosteroids (diprospan, dexamethasone, Kenalog, hydrocortisone) is indicated. The anti-inflammatory effect is achieved by introducing a corticosteroid into the cavity of the diseased joint from 3 to 5 injections with an interval between injections of 5-7 days.

    Dosages for 1 administration:

  • diprospan 2 mg (1 ml),
  • dexamethasone 4 mg (1 ml),
  • Kenalog 40 mg (1 ml),
  • hydrocortisone 125 mg (5 ml).

    PHYSIOTHERAPY FOR ARTHROUS DISEASE

    In the complex conservative treatment of patients with arthrosis disease, physiotherapeutic methods of influence occupy one of the leading places. The possibilities of modern physiotherapy are quite large. Physiotherapeutic treatment methods should be used taking into account the age of the patient, concomitant diseases, clinical picture, severity of joint damage and the tasks being solved. The main objectives of physiotherapy when carrying out preventive and rehabilitation measures for this category of patients are:

    1. Stimulation of the adaptive-trophic functions of the sympathetic nervous and sympatho-adrenal systems;

    2.Increasing blood and lymph circulation, increasing biochemical processes, improving metabolism and trophism of tissues of the diseased joint;

    3. Providing analgesic, anti-edematous and anti-inflammatory effects;

    4. Acceleration of the processes of resorption of tissue metabolic products, edema, infiltrates, exudates, hemorrhages;

    5.Increasing the body's defenses and immunomodulatory effect;

    6.Preventing atrophy, muscle shortening and reducing their spasm;

    7. Strengthening the desensitizing and detoxification effect;

    8.Improving regeneration processes, preventing the progression of the disease and restoring the function of the affected joint.

    When conducting physiotherapeutic treatment, it is necessary to take into account the mechanism and effect of each of the prescribed therapeutic factors.

    This section is the most developed when prescribing the treatment of deforming osteoarthritis, they account for more than 60% of all procedures.

          If the load on the musculoskeletal system weakens, excess synovial fluid is absorbed by the lymphatic vessels of the cartilage. Thanks to this mechanism, there is virtually no friction in the joint even under significant loads.

          An imbalance leads to the fact that little or, conversely, too much synovial fluid is produced. Both are harmful to the full function of the joint.

          Why does synovial imbalance occur in the joint capsule?

          If the joints lack lubrication, the ends of the bones begin to rub against each other. This phenomenon is fraught with premature wear or irreversible destruction of the articular surfaces.

          These processes are accompanied by discomfort, crunching and pain and can even lead to partial or complete limitation of mobility in the affected joints.

          Here are the main reasons for the reduction in the volume of synovial exudate:

        • decreased immunity;
