Inflammation of the veins of the lower extremities. Inflammation of the veins of the lower extremities is more common than the upper ones. The most common cause of inflammation of the veins of the lower extremities is varicose veins that have not been treated. Factors contributing to inflammation of the veins in the legs are:
- slow blood movement, which is associated with a general slowdown in blood flow in the body;
- bed rest for a long time;
- compression of tissue around the vein;
- increased blood clotting, both congenital and acquired as a result of cancer, hormonal imbalance, taking certain medications, infectious diseases;
- damage to the venous wall as a result of mechanical or surgical impact, placement of intravenous catheters and the introduction of chemical solutions.
Inflammation of the veins of the upper extremities. Inflammation of the veins in the arm most often appears as a complication after venous catheterization. Vein problems may depend on the size of the catheter, where it is inserted, the type of fluid given, and the duration and frequency of the solution. In addition, the cause of phlebitis on the arm can be a catheter infection caused by staphylococci, enterococci, Candida, etc.
Signs of the disease will depend on the location of the blood clot and the form of the disease. With thrombophlebitis, swelling in the legs appears, heat and heaviness in the legs appear, skin color changes, severe pain in the calf muscles, swelling and hardness of the veins, and increased body temperature. However, often inflammation of the internal veins develops without any symptoms or very weak ones, and this leads to serious complications and even death.
Thrombophlebitis is treated in two ways: conservative and surgical. Strict bed rest is usually prescribed to prevent embolism and the spread of infection.
The limbs should be placed in an elevated position using a splint to improve venous drainage and reduce pain and swelling. In this case, you should drink a large amount of liquid - 2–3 liters per day.
With superficial thrombophlebitis, the patient is allowed to sit and turn, hold the leg in a horizontal position, and apply warm compresses. To reduce pain and improve blood circulation, a lumbar novocaine blockade is performed.
Conservative treatment includes:
— physiotherapy (sollux, ultraviolet irradiation, infrared rays);
- anticoagulants to reduce blood clotting;
- drugs that reduce the amount of prothrombin in the blood and prevent the formation of new blood clots (phenylin, sipkumar, dicoumarin, neodicoumarin);
- hirudotherapy (treatment with leeches) is used only for acute thrombophlebitis if the patient has contraindications to taking anticoagulants;
- fibrinolytic drugs that lead to the destruction of blood clots at an early stage (fibrinolysin, streptokinase, urokinase, trypsin, chymotrypsin).
Surgical methods include vein ligation, venectomy, dissection and excision of thrombosed nodes of the superficial veins.
