Trental (Trental Pentoxifyllinum) is a vasodilator vasodilator , blocks adenosine receptors, increases cAMP of cellular structures, stimulates microcirculation, and enhances the rheological characteristics of blood.

Mechanism of action of the active substance

The formation of blood pressure and oxygen saturation of tissue is carried out by microvessels and capillaries (arterioles, venules). Maintaining microcirculation between them: collaterals (lateral vessels).

Calcium ions influence the interaction of the actin protein and myosin, ensuring the contractility of smooth and striated muscles. The passage of the blood duct narrows, the smooth muscles begin to contract intensely, and antispasmodic contractions (vasospasms) occur.

Trental blocks enzymes that hydrolyze the phosphodiester bond (PDE), changing the calcium content inside the cell matrix. The quantitative composition of calcium is influenced by cyclic adenosine monophosphate (cAMP), formed from adenosine triphosphoric acid (AMP) under the influence of adenylate cyclase enzymes. The active element (Pentoxifylline) blocks enzymes, increases the level of accumulation of internal cellular cyclic adenosine monophosphates, reduces the amount of calcium, relaxes muscle walls, increases vascular lumen, stabilizes blood flow and oxygen supply.

  • Pharmacokinetics of the drug

    Trental is characterized by accelerated absorption. The prolonged form of the tablets stimulates the constant movement of the active elements and uniform absorption of the sorbate. When administered orally, the active substances pass through the liver, being converted into pharmacological metabolites that are 5 (8) times greater than pentoxifylline. Metabolites are distributed evenly and are mainly excreted by the kidneys within 4 hours. With pathologies of the genitourinary system, the rate of excretion of metabolites decreases.

    Directions for use and recommended dosages

    Trental, release form - tablets, instructions for use:

    Average oral dosage: 1 (0.1 g) tablet three times a day, during or after meals. According to indications, over time the dosage increases to 0.2 g (2 tablets) every 6 hours. The maximum single dose can be no more than 0.4 g, the daily dose should not exceed 1.2 g.

    Invasive routes of administration and doses:

    IM injections: 5 ml (or 10 ml) ampoules; injections are given two to three times a day. Slow, deep administration of the drug is recommended.

    Parenteral (boost) : 100 mg (1 ampoule) dissolved in 20 ml of sodium chloride, administered very slowly over at least 7 minutes, with the patient in a horizontal position. The procedure is repeated 3 times a day.

    Parenterally (drip) is prescribed in a ratio of 10 ml (0.2 g of pentoxifylline) concentrate, previously diluted with 250 ml of sodium chloride or Ringer's solution. The drip is placed intravenously for 2-3 hours, with an interval of 10-12 hours. The period of infusion of a solution with 0.1 g (5 ml) of pentoxifylline should last at least an hour.

  • For cardiac pathologies, the volume of injected solution is reduced. An infuser is used to control drip administration.
  • The duration of infusion is extended to 24 hours for gangrene and severe brain damage.
  • In the interval between droppers, 2 pieces of Trental (400 mg) are prescribed orally. For a one-time daily infusion: 3 tablets per day, of which 2 are taken in the middle of the day and 1 in the evening.

    Symptoms of overdose are:

    • Reduced blood pressure.
    • Nausea.
    • Dizziness.
    • Feverish condition.
    • Hot flashes.
    • Fainting.
    • Convulsive syndrome.
    • Black or dark brown vomit (stomach bleeding).
    • For uncomplicated symptoms, it is recommended to urgently take absorbents to cleanse the body: activated carbon in a ratio of 2 tablets per 10 kg of body weight.

      The main actions in case of overdose: normalization of blood pressure, restoration of respiratory functions and prevention of seizures. If necessary, seizures can be stopped with Diazepam (invasively).

      In case of gastric bleeding, convulsions, fainting and severe hypotension, urgent hospitalization is required. Self-treatment in these cases is unacceptable.

      Indications for use

    • Discirculatory encephalopathy (DEP) is damage to the arteries and arterioles in the brain.

    • Contraindications and restrictions

    • Intolerance to components, hypersensitivity to pentoxifylline.
    • Hemophilia.
    • Hemorrhagic stroke.

    • Contraindications for injections:

      The official summary does not recommend taking Trental during pregnancy due to the lack of clinical trial data. The medicine is prescribed during the perinatal period if the benefit outweighs the potential harm to the health of the fetus.

    • From the second trimester of pregnancy, drip administration of the drug Trental is prescribed for placental insufficiency and Gestosis (a complication of the normal course of pregnancy).     • Genitourinary system : hematuria, renal failure.
    • Uncharacteristic complications: anaphylactic shock, Stevens-Jones disease, Lyell's syndrome.
    • A large list of side effects requires constant monitoring of blood pressure levels, the functioning of the circulatory system, the functionality of the genitourinary organs, liver and gastrointestinal tract.

      The following diseases and conditions require special monitoring:

    • Diabetes mellitus: the use of hypoglycemic medications requires a reduction in the standard dosage of pentoxifylline. High levels of components can provoke hypoglycemia. To normalize the condition, the dose of antidiabetic drugs is reduced (strictly according to the doctor’s indications).
    • After surgery, the drug is taken taking into account hemoglobin and hematocrit levels.
    • Take with caution: severe atherosclerosis of the head and coronary vessels; arrhythmia; hypotension or labile blood pressure. In these cases, it is advisable to prescribe reduced doses of the drug; if necessary, increases are made gradually and under constant medical supervision.

          Trental (INN - pentoxifylline) is an original drug from the group of vasodilators from the Indian division of the world famous pharmaceutical company Sanofi. The first use of trental dates back to 1972, when it was used to treat intermittent claudication. In addition to the vasodilator, the drug has an antiplatelet (prevents platelets from adhering to each other) and angioprotective (protects blood vessels) effect. Due to its ability to improve blood fluidity, reduce platelet aggregation, stimulate capillary circulation and make red blood cells more elastic, trental has found wide use in various circulatory pathologies. The main “weapon” of trental, pentoxifylline, is very close in its chemical “pedigree” to theophylline and theobromine. All its pharmacological advantages are due to the exclusive possession of the secret inhibitory effect on type 4 phosphodiesterases, which entails an increase in the concentration of cAMP in platelets and ATP in erythrocytes against the background of a decrease in the content of calcium ions in cells. Numerous clinical studies conducted in accordance with all the rules of evidence-based medicine indicate the ability of trental to have a purely positive effect on microcirculation and blood circulation in general.           When answering the question whether to choose Pentoxifylline or Trental, you should take into account the quality of production of both products. The Russian analogue is of good quality considering its low cost. But Pentoxifylline is also produced by Ukrainian companies, where its price is even lower. The effect of this medication is much lower. Trental is produced in Indian and Swiss factories, using modern equipment and using modern materials. The quality of cleaning is significantly higher than that of Russian and Ukrainian analogues. The medicinal base of the tablets causes fewer allergic reactions. Trental injection solution undergoes better quality control.

          The mechanism of action for both drugs is the same. While taking Trental, there is a faster restoration of blood circulation than with treatment with Pentoxifylline. This is confirmed by numerous studies that were conducted for imported products. Domestic Pentoxifylline did not have such studies.

          For oral use, there is no significant difference between Trental and Pentoxifylline; both drugs can be used. But for intravenous use, it is still better to prescribe Trental, as a better and more effective medicine.

          There are also no differences in ease of use, but Trental is available in a dosage of 400 mg, while Pentoxifylline does not have such a dosage. Therefore, if an increased dose is required, it is more convenient to take a Trental tablet rather than 4 Pentoxifylline tablets. This is especially convenient when the patient requires a long course of treatment.

          The disadvantage of Trental is its relatively high cost. Not every patient can afford to purchase an expensive medicine and replaces it with an analogue called Pentoxifylline, which is cheaper.
