Tophi in gout are accumulations of uric acid crystals in the form of granular granules. Localization of tophi mainly occurs in soft tissues located:
This formation is a specific sign of gout, a disease in which the level of uric acid is elevated.
The word tofus means “porous stone”. The growth, tophi, is a pathological area of compacted tissue. This process is characteristic of gout. If the disease lasts more than 5 years and is accompanied by high hyperuricemia (in this condition, the level of uric acid is much higher than normal), tophi are formed.
The sizes of tophi vary from a few millimeters to 2 cm (see photo). Tophi has a yellowish tint and does not cause pain. The consistency of the contents in the granule can be powdery, creamy or liquid.
If the tophi becomes inflamed, it may develop a fistula. In such a situation, the contents of the tophi liquefy and flow out, and an ulcer forms at the site of the ruptured fistula. Over time, the wound either heals on its own or breaks out again.
Gradually, the periarticular bursae are also involved in the inflammatory process, as in the photo. This pathology is called bursitis. We can talk about tenosynovitis when the tendons also undergo pathological changes. All these conditions are accompanied by severe pain.
Note! Gout, which results in the formation of tophi, occurs slowly. Treatment takes quite a long time. Formations called tophi appear only several years after the first signs of the disease appear. If the patient uses diuretics or suffers from kidney disease, granules may form much earlier.
In addition to the legs, similar tophi can be observed on the fingers, ears (auricular tophi), in the nasal cartilage, and in the elbow area. Typically, tophi is located on the skin, but there are cases of visceral - internal or bone marrow localization.
In other words, in addition to visible external seals, tophi can also be located on internal organs. Gouty type - this type of tophi is quite difficult to detect.
Uric acid occurs as a result of the breakdown of purines and its presence can be found in any organism. The normal level of uric acid for an adult is 1 gram. As a result of metabolic processes, purines are synthesized; in addition, these elements are supplied with food.
In blood plasma, uric acid is found in the form of sodium urates or otherwise free salts. If the level of uric acid increases, hyperurecemia develops, which after some time leads to gout.
The initial stage of the disease is characterized by an asymptomatic course. Most people during this period are not even aware of existing violations. The first clinical signs usually appear only after several years.
Throughout this period, urate crystals are deposited in tissues and joints, causing irreparable harm to the body.
The same tophi can appear in cancer of the bone marrow (myeloma) and some kidney pathologies.
The development of gout is facilitated by metabolic disorders in the body, and tophi is present everywhere.
You can see what the formations look like in the photo. Here are their main features:
Nodules are the main symptom of gout. Similar formations are observed when
rheumatoid arthritis or arthrosis deformans and can form in different parts of the body (see photo). Localization of granules can be:
If gouty formations are detected, the tophi tissue is examined. Their location and structure in different organs is approximately the same.
The central part of such a tumor consists of crystalline contents. The tissues located around are subject to dystrophic and inflammatory processes. The doctor can determine the severity and duration of the disease by the structure of each tissue.
Severe pain in the joints is a reason to consult a doctor, because these sensations may signal the development of gout in the body. The pain can last for several months and forces a person to constantly take painkillers.
A similar course of the disease is typical for the period of exacerbation. At this moment, the joints greatly increase in size, which becomes noticeable even to the naked eye. This is roughly what they look like in the photo.
New growths may initially have a soft structure, but harden over time. Nodes are formed either singly or in groups. The skin over them has a bluish tint. Sometimes the tubercles resolve on their own, for no apparent reason, but they can break through, which gives the skin an unaesthetic appearance, as in the photo.
Treatment for gout is based on lifestyle changes. The patient must adhere to a specific diet and stop drinking alcohol. The treatment regimen is selected individually in each specific case. The doctor pays attention to the general condition of the patient and test results, regardless of the type of disease, because it could be idiopathic gout.
Treatment is as follows:
To reduce the size of gout nodes, the patient is recommended to take:
So, we settled on the fact that gout is not something to joke about and this disease is scary not only due to wild pain in the joints, but also other complications. We won’t talk about chronic renal failure and the ensuing consequences for now; I don’t have any personal experience, pah-pah-pah, but we’ll talk a little about tophi, which are a consequence of this disease.
From medical sources:
Tofus is the general name for foci of pathological compaction of the subcutaneous tissue.
Gouty tophi is the deposition of uric acid crystals in soft tissues in the form of peculiar granulomas in gout. Tophi are usually localized in the subcutaneous tissue above the joints of the hand and foot, elbow joints and ears, on the extensor surface of the forearms, thighs, legs, Achilles tendons, and on the forehead.
It is a specific sign of gout - they are formed with high hyperuricemia and the duration of the disease over 5-6 years. They are painless nodular formations ranging in size from a few millimeters to 1-2 cm, predominantly yellowish in color. During attacks, their contents can liquefy and be released through fistulas.
Gradually, inflammation may occur in nearby periarticular bursae (bursitis) or tendons (tenosynovitis), accompanied by pain.
You can see what tophi look like by typing the corresponding query in any search engine. I’ll say right away that the picture is unpleasant. But, I think, the search engine results contain just the most “outstanding” cases of tophi formation, and in fact the picture is not so sad. In principle, my case somewhat confirms this assumption. But let's talk about everything in order...
Gout, or rather its first attack, was very painful for me and for a very long time: for a month and a half I was not relieved by severe pain and only periodic short subsidences allowed me to rest from taking painkillers. I wrote about this in detail here. When it was all over (I mean the exacerbation), the visible consequences that remained were a rather noticeable enlargement of the joint of the big toe of my right foot. Not an obvious bump, but if you put your two feet side by side, you can see the difference. After looking at the pictures of gouty joints and tophi, I thought that I got off very lightly, given the severity and duration of the exacerbation.
6 years have passed since this nightmare, but there have been no more exacerbations, pah-pah-pah again. This is mainly due to lifestyle changes and adherence, albeit partial, to diet. I didn’t even think about tophi; I thought that they could only appear with advanced gout, but, alas, I was wrong.
Somehow, about four years after the first onset of gout, I broke all the prohibitions and “pulled up” well. I must say very well... Considering that for 4 years I practically did not drink alcohol, this episode could be called an alcoholic disaster. In short, everything is in the best traditions of its “brave” alcoholic past. Hangover was simply mind-blowing! I was “sick” for two days and drank green tea and juices. No manifestations of gout in the form of incipient pain in the joints were noticed, gradually the hangover dissipated and the only bad “aftertaste” was a feeling of discomfort in the area of the masticatory muscle at the very corner of the jaw on the left side. I made a mistake on some lymph nodes or inflammation of the salivary gland, intoxication was good. Gradually, the feeling of discomfort almost went away, but under the skin at the corner of the lower jaw there remained a noticeable compaction to the touch. Looking at myself in the mirror, I noticed that a slightly noticeable asymmetry of the lower jaw had appeared. But this did not bother my education in any way, and I postponed the “showdown” with this bullshit for calmer times.
This is what it looks like today:
It's not very visible in the photo, but in real life the asymmetry is more noticeable. It is quite clearly visible that the lower salivary gland is slightly enlarged (yellow arrow).
Here's a side view. On the right side, the angle of the jaw does not have such a noticeable “angular bend”
Considering the topic of this post, many have already guessed that we are talking about tophi. But, gentlemen, you can’t even imagine what I had to go through!
After some time, this formation began to sometimes, not very often, about once every six months, become inflamed. These inflammations did not cause any particular discomfort; it did not hurt much, but it was unpleasant. The skin over the bump turned slightly red, and some tension was felt in the salivary glands. Naturally, I immediately thought about inflammation or some kind of chronic disease associated with the salivary glands. It took me three years and several periodic inflammations to finally start diagnosing this formation. It’s not that I didn’t want to do this, the reason is different: I live in a provincial town and we simply don’t have an oral and maxillofacial surgeon, only in the regional hospital.
But, nevertheless, another inflammation of the lump forced me to move in the right direction: I finally ended up with an oral and maxillofacial surgeon, who, after feeling the lump, gave a preliminary diagnosis: salivary gland adenoma! Cut and only cut - that was the verdict. Here's to you, grandma, and St. George's Day... Adenoma is not a completely harmless thing. And is it an adenoma? That is the question! I did an x-ray and an ultrasound - the conclusions were that it was supposedly an adenoma of the parotid salivary gland. Kick! Having read on the Internet what this threatens and taking into account my suspiciousness that developed during the “first coming of the VSD,” I completely lost heart and once again “went through options for leaving for another world.”
Then, while waiting for an appointment with the head of the department of maxillofacial surgery, thinking that it would not be superfluous, I also did an MRI of the lower part of the skull and neck, which showed that the formation was not associated with either the lymph nodes or the salivary glands, but how - would “hang” in the thickness of the masticatory muscle. The diagnostician describing the MRI results was very young; there was little hope that he was right and that it was not an adenoma. Again, if not an adenoma, then what. Cyst? Cancer tumor? Damn, it got even worse!
I won’t drag it out, there are already “a lot of letters.” At the appointment with the department head, the MRI scan was very useful, by the way. The old man-surgeon spent a long time feeling me and picking at my jaw, looking at the pictures, reading the ultrasound report. My silent question was answered almost immediately - NOT ONCOLOGY 99.9%! Already good! But there remains an adenoma that must be removed, and the operation, as it turned out, is not the easiest.
After much thought, the doctor finally asked a historically significant question: “Young man, have you by any chance heard anything about such a disease as gout?” Well, of course, we haven’t heard, very much so! And then he said with relief: “Then I won’t doubt it - it’s tophi! It’s just that, looking at you, you can’t say that you’re sick with anything at all, much less gout.” It was a compliment, I won't hide it. Especially considering my “set” of illnesses and phobias. It turns out that from the outside it’s not all that bad :)
That's how I ended up in the 1% of those whose tophi form in soft tissues near bones and tendons. Periodic inflammations immediately “fall into place”, which are not characteristic of adenoma, but, as is known, are characteristic of tophi. So my gout didn’t end with just memories... Oh well, I remember, I remember! Gout is incurable and insidious, it can wake up at any moment, just give it some slack and it will immediately take advantage of it!
Gout is a disease that is caused by disorders of purine metabolism. The disease is accompanied by the deposition of salts in various organs and, above all, in the joints.
In the later stages of the disease, patients may develop characteristic nodules on the skin - tophi. They are formed due to deposits of crystalline uric acid salts limited to connective tissue. And if, in addition to these salts, calcium salts are deposited, then a disease such as skin calcification may develop.
As a rule, at least 5 years pass from the onset of the disease for tophi to appear. However, in patients suffering from juvenile gout, as well as those with kidney disease or taking certain types of diuretics, tophi may appear much earlier.
Gout has been known to people since ancient times; a description of this disease is found in the treatises compiled by Hippocrates. However, the most significant milestone in the study of this disease was the discovery of uric acid and the discovery of urates in gouty nodes (tophi). Gout is a common disease; the majority of patients are men over 40 years of age. Gout, like monilethrix in women, most often develops after menopause.
Uric acid is present in the body of every person, as it is a breakdown product of purines. The normal level of uric acid in an adult is 1000 mg, the circulation rate of this substance is 650 grams per day. Purines come from food and are also synthesized in the body itself in the metabolic processes of nucleotides.
Uric acid is present in the blood plasma in the form of a free salt - sodium urate. When the level of uric acid exceeds the level, hyperurecemia occurs, which creates a high risk of developing gout.
Many patients with hyperuricemia are asymptomatic, so people often do not realize that they may soon develop an attack of gout. Clinical manifestations of the disease usually appear after several years. As the disease progresses, urate crystals are deposited in the tissues, which leads to the development of tophi. Gout can also cause the development of panniculitis.
Gouty nodes or tophi look like dense tumor-like formations. The appearance of these formations is observed in the later stages of the disease, when gout from an acute form becomes chronic.
Tophi have sharply limited edges, the consistency of the node is very dense. The skin covering the tophi is rough, and hyperkeratosis is also observed. If the nodes are located close to the surface, then the yellow-white color of the tophi appears through the skin.
Tophi can have different sizes, from very small to gigantic, comparable in size to a large apple. The depth of tophi can also vary significantly. Sometimes the node forms intradermally, but a visceral or bone marrow location of the tophi is also possible. Tophi are often confused with rheumatoid nodules.
The location of tophi may be the skin.
Tophi can form on different parts of the body. The location of the gouty node can be:
However, the most likely places for the formation of tophi are considered to be:
An extremely rare variant of the localization of tophi is:
In most cases, the formed tophi do not develop, maintaining a static position for a long time. However, as gout progresses, the nodes may begin to grow rapidly. The size of tophi can increase so much that the underlying tissue is destroyed. Clinically, this condition is manifested by the occurrence of severe pain and inflammatory processes in the tissues of the organ on which the tophi are located.
The nodes can spontaneously open with the formation of fistulas. A yellowish-white mass is released from the fistulas. The released mass may have a liquid, creamy or crumbly consistency. At the site of the fistula, a superficial ulcer may form, which will periodically scar and erupt again.
Tophi are not only a diagnostic symptom of gout, but also a sign of the duration and severity of the disease.
As a rule, multiple large tophi develop in patients who have suffered from gout for at least 6 years and have severe hyperuricemia. With very high levels of urate in plasma, tophi may appear as early as the 3rd year of the disease.
The basis for diagnosing tophi and gout are laboratory tests and external examination of gouty nodes.
The main signs of tophi:
The treatment regimen for gout is selected individually, depending on laboratory test data and the general condition of the patient. Treatment goals:
Patients are advised to follow a diet. Alcohol and foods rich in purines (offal, river fish, mushrooms, legumes) are completely excluded. Physical education and any type of physical activity is very useful. For patients with excess weight and cellulite, a low-calorie diet is recommended, but fasting during gout is strictly prohibited.
If tophi become large and have an adverse effect on surrounding tissues, surgical removal is possible.
To treat gout and reduce the size of tophi, traditional healers recommend:
The prognosis for gout with the formation of tophi depends on the intensity of urate excretion by the kidneys. With a good level of excretion, the disease takes a long time, but is quite easy, the tophi remain minimal in size.
In severe cases of the disease, the formation of massive tophi is possible, which destroys the joints and underlying tissues. With this course of the disease, the patient may become disabled after a short time.
If there is a person with gout in the family, close blood relatives are recommended to be screened for hyperuricemia. If an increase in the level of uric acid salts is detected, for the purpose of prevention it is recommended to undergo a course of preventive treatment. In particular, a long-term course of allopurinol may be prescribed. As a result of long-term use of the drug, a drug rash may appear. In addition, with hyperuricemia, patients are recommended to exercise, walk, and do as much physical activity as possible, as this helps to quickly increase the excretion of uric acid.
Tophi are chalky nodules, foci of compaction, one of the characteristic signs of gout. Unaesthetic formations are uric acid salts that accumulate in the joint tissue due to improper metabolism. More than a quarter of gout patients experience the appearance of tophi in the area of small joints, elbows, and on the ears.
The tubercles not only worsen the appearance, but also impair the mobility of the joints. Not all patients know that tophi are not as harmless as they might seem at first glance. Seals are not just a cosmetic defect: if left untreated, the tubercles grow, destroy cartilage tissue, and often provoke ulcers and skin necrosis.
Tophi is an accumulation of uric acid salts in close proximity to the skin. When too much urate accumulates, it affects not only the bursa (bursa), synovial fluid, tendons, and cartilage, but also surrounding tissues. Seals appear in the layers of the epidermis and dermis.
Tophi are tiny crystals of uric acid salts, produced in excess quantities, but not excreted from the body. Tophi disrupt the structure of peripheral tissues and fill the gaps.
In addition to urate crystals, granulomatous tissue was detected in the gouty tubercles. The more uric acid salts accumulate, the more actively harmful, unesthetic new growths grow. The size of gouty nodules ranges from 2–3 mm to 2 cm.
The main factor is excessive production and poor excretion of salts of harmful uric acid. When the cycle is disrupted, local accumulation of urates occurs in certain areas. This is how gouty nodules or tophi are formed.
The more negative factors act, the faster unsightly, dangerous tubercles form. Violation of the drinking regime, excessive consumption of foods with purines, bad habits (addiction to alcohol, cigarettes) accelerate the growth of harmful urate-based seals.

In most patients, tophi do not appear in one day; many do not immediately pay attention to gouty nodules. With a mild degree of metabolic pathology, the subcutaneous tissue is slightly modified, there are no “bumps” yet, but the negative process has begun. In places where urate actively accumulates, white spots appear, and the skin often becomes discolored.
Gradually, the spots become more noticeable, yellowish, and subcutaneous “bumps” appear. Nodules of different sizes rarely hurt, but as they grow they put pressure on neighboring tissues, experience shoe friction, and develop discomfort and pain.
Location on the feet (in the area of the Achilles tendon), on the toes provokes constant compression, friction of the tubercles, damage to the skin, and increases discomfort. With an abundant accumulation of urates, the skin breaks through, the curdled or pasty contents come out, and a fistula appears at the site of the tophi. The area does not heal for a long time, and there is a high risk of infection of epithelial and subepithelial tissue.
Urate deposits most often accumulate in areas with poorly developed vascular networks. Body temperature in these areas is lower than in other parts of the body; the temperature difference accelerates the sedimentation of uric acid crystals.
In most patients, tophi are located in the following areas:
Patients diagnosed with gout should be aware of the negative impact of gouty nodules. Seals are an accumulation of harmful salts - urates. With a high concentration of uric acid in tophi, there comes a time when the component enters back into the blood, worsening the condition of the body. The more uric acid the body receives from gouty tubercles, the higher the risk of exacerbation.
If left untreated, tophi grow and cover the fingers, toes, ears, and elbows. Formations destroy skeletal structures, worsen the condition of cartilage, tendons, joints, and skin. In advanced cases, with severe gout, the mobility of the limbs is lost, and the patient receives a disability group.
The appearance of tophi is a signal of the active production and accumulation of monosodium urate in the form of small crystals in various tissues and organs. You cannot watch how gouty lumps grow and take over new areas: inattention to the manifestations of gout provokes complications.
The fight against tophi includes several elements:
A rheumatologist will develop a treatment regimen and prescribe uricosuric or uricodepressive medications. Blood and urine tests will help determine what process is occurring: excessive accumulation of urate or slow excretion of uric acid salts.
In the initial stage, the tubercles on the toes and hands, elbows, and ears are small; home methods will help to cope with them. You can get rid of gouty nodules if you understand the reason for the appearance of unsightly formations. As long as uric acid salts accumulate at an accelerated pace, there will be no benefit from using traditional recipes. Along with therapeutic baths, wraps, and application of a special mixture, the patient must follow a gout diet, drink more water, and take medications prescribed by the doctor.
Home methods for treating tophi:
For gout attacks, the doctor will prescribe:
During the period of remission, other methods are used to prevent attacks:
In case of severity of the case, active growth of tophi, damage to the deep layers of the skin, doctors recommend removal of unesthetic formations. If this is not done, the cartilage will begin to deteriorate, and long-lasting non-healing ulcers will appear in the area of the tubercles.
Surgery is mandatory in the following cases:
Under no circumstances should you cut out the tubercles with a blade or try to cauterize the seals. Doctors will offer the most gentle method with minimal trauma to the skin. Often, a rheumatologist recommends surgery to remove significant amounts of uric acid accumulated in large tophi.

It is easier to prevent the development of gout than to constantly fear the next attack. The disease occurs in mild and severe forms. The degree of joint damage largely depends on the patient’s compliance with nutritional rules and other preventive measures.
Pay attention to the recommendations of doctors:
The following is a video on how to treat gout and gouty tophi:
In medicine, the term “tophi” refers to nodular subcutaneous growths characteristic of the advanced stage of a disease such as gout. This disease occurs when the metabolic process in the body is disrupted. The failure occurs due to the increased amount of purines that fill a person’s daily diet - often poor nutrition is a causative factor in the formation of gout.
Purines promote the deposition of uric acid, which is converted into crystalline compounds and subsequently, in the chronic course of gout, tophi appear - on average, for their manifestation it takes at least 5 years of “neighborhood” with the disease.
A dense tumor-like ball under the skin is a tophi. The location of these formations may be:
Most often, tophi are localized on the ears, in the area of the elbow joint, on the hands and feet. Less likely, but such cases have been recorded in practice; they can be found on the skin of the face, foreskin and scrotum, as well as on the cornea and sclera.
Gouty nodules stand out in contrast with their borders and increased density. At the site where tophi has formed, the skin will be rough to the touch and even have growths of the stratum corneum (hyperkeratosis).
If the formation is located in the subcutaneous tissue (closer to the surface), then the distinguishing feature will be the whitish color of the nodule with a yellowish tint, as in the photo. Although the depth of the tophi may vary. They are sometimes confused with nodules of rheumatoid origin, as the sizes can vary from small to large in diameter.
Often the formed tophi retains its static parameters. By this time, the patient is already taking all measures to cure gout. However, if treatment is delayed, the disease progresses and there is a risk of tumor nodes growing to enormous sizes. Moreover, due to growth, tophi begin to destroy nearby tissues. The person experiences strong painful sensations.
Under the pressure of the resulting load, the gouty nodes seem to crack (open on their own), forming fistulas (fistulas). A kind of liquor is released - a mass that can have different consistencies - creamy, liquid or crumbly. Fistulas, in turn, can become covered with ulcerative wounds, which scar and then rupture again.
The tophi itself is already a sign of advanced gout, but it is recommended to conduct laboratory tests after an external examination to make an accurate diagnosis.
The drug treatment regimen is selected by the attending physician individually in each case, after studying the medical history and the stage of its pathological development.
General recommendations for gout, including the occurrence of tophi, is dietary nutrition, following which the patient reduces the content of uric acid in the body. Foods high in purines - legumes, meat and offal, fish (mainly river) are excluded from the daily diet.
Often patients with gout suffer from excess weight and need to get rid of it. However, a diet for them does not mean complete fasting; it will be enough to arrange fasting days and drink alkaline mineral waters.
If the tophi have nevertheless grown to large sizes, which destroy tissue, and their external and painful state prevents a person from living fully, a decision is made to resect (remove) the formations surgically. The prognosis is favorable - relapses of tophi are rare, provided proper treatment.
With a disease such as gout, traditional medicine treatment methods play a large and effective role. Firstly, gout cannot be cured forever, but with the help of traditional methods of treatment you can support the body, since pharmaceutical drugs often have many side effects. Therefore, by choosing the right herbal decoction or a set of procedures from traditional medicine, you can successfully treat and prevent gout.
Of the medicinal decoctions that are taken for gouty tophi, the following preparations are popular:
Take in equal shares:
After mixing the components of one or another collection, for brewing you need to take two tablespoons, which is poured with half a liter of boiling water. Covering the container, leave the collection to infuse until the morning. Next, this decoction is diluted with boiling water at the rate of 2 tbsp. l. for 0.5 water. Take half a glass several times a day. The course of taking the decoction is at least 2 months.
Tophi in gout are accumulations of uric acid crystals in the form of granular granules. Their concentration is formed in soft tissues. Localization of crystals occurs over the joints
feet, on the extensor surface of the thighs, shins, and Achilles tendons. Tophi is a specific sign of gout, in which uric acid levels are elevated.
The word tofus translates as porous stone. It refers to the focus of pathology of compacted subcutaneous tissue. This process occurs during gout. Nodular formations are formed during a long course of the disease (more than 5 years) and high hyperuricemia, which is characterized by a level of uric acid that is higher than normal.
The size of tophi ranges from several millimeters to 2 cm. They have a yellowish tint. The nodules do not cause pain. When the formations become inflamed, fistulas may appear in them. Then the internal contents liquefy and can flow out through such channels. The consistency of the substance contained in the granule can be creamy, liquid or crumbly. An ulcer may form at the site of the fistula rupture. Over time, the wound either heals on its own or breaks out again.
The periarticular bursae may also gradually become inflamed. This pathology is called bursitis. If the tendons are affected by pathology, it is called tenosynovitis. These diseases are accompanied by severe pain.
The development of gout, as a source of tophi, occurs slowly. Treatment takes place over a long period of time. Several years after contracting gout, neoplasms called tophi appear. With some types of gout and use of diuretics, as well as kidney disease, granules may appear earlier than the usual period of six years. In addition to the legs, such nodes are found on the ears, fingers, nasal cartilage, and elbows. The bumps are usually located in the skin, but there are cases of visceral (internal) or bone marrow localization. That is, in addition to visible external seals on the body, they can be found on internal organs. In such cases, it is especially difficult to detect them.
Uric acid is a breakdown product of purines and is present in every body. In an adult, its normal content is 1000 mg. As a result of metabolic processes in the body, purines are synthesized. They also come with food. Uric acid in the blood plasma is in the form of sodium urates, the so-called free salts. When the level of uric acid increases above normal, hyperurecemia is formed. Over time, it leads to gout.
The onset of the disease is asymptomatic. Many people are not even aware of disorders in the body. Clinical manifestations of the disease usually become apparent after several years. All this time, urate crystals are deposited in joints and tissues, causing harm to the body. Tophi can also form in bone marrow cancer (myeloma) and certain kidney diseases.
Gout is caused by metabolic disorders in the body. When the disease occurs, uric acid salts are deposited in various organs, especially in the joints. The disease is more common in men, especially after forty years. Tophus is a characteristic symptom of a person suffering from gout for a long time. And that means he has high levels of uric acid in his body. Depending on the size and number of nodes, the severity of the underlying disease is determined.
The first manifestation of gout can be an acute attack. Only after a while do tophi form. Gout formations look like nodules. They feel firm to the touch. Their edges have clear outlines. The contents of such a node have a dense consistency, sometimes rocky. The surface of the pathological formation has roughness. When located shallowly, they are visible through the skin, and an accumulation of their contents, which is white-yellow in color, is visible. Fistulas can form on tophi, through which the nodes are opened. Then their milky inner filling flows out. Such fistulas usually do not become infected.
Tophi are a clear sign of gout. Such neoplasms also appear in deforming arthrosis and rheumatoid arthritis. Tophi are characterized by formation in various parts of the body.
The places where granules form can be:
When identifying gouty nodes, doctors examine the tophi tissue. Their structure and distribution in different organs is approximately the same. The center of such a tumor consists of crystalline contents, and the surrounding tissues undergo degenerative processes. The tissue around the node becomes inflamed. Based on the structure of each tissue, the specialist determines the severity of the disease and the period of progression of gout.
Gout manifests itself as severe pain in the joints. They can last for several months. This forces patients to constantly take painkillers. This course of the disease is typical for the period of exacerbation. At the same time, the joints increase in size, which is visible to the naked eye.
Tophi may be soft at first and harden over time. They can form individually locally, or in groups of several nearby. The color of the skin above them does not change. It is bluish in color. Sometimes they resolve on their own. They can break out or exist for a long time, which gives an unsightly appearance to a person.
With such a pathology, you have to change your lifestyle. You need to go on a special diet and stop drinking alcohol. The doctor selects a treatment regimen for gout, as the main cause of tophi, individually for each person. The test results and the patient’s overall condition are taken into account.
To reduce the size of gouty nodes, as well as treat gout, it is recommended:
to contents ^
The chances of a normal life with gout with the presence of tophi depend on how intensively the kidneys remove urate from the body. With good kidney function, the disease is mild, although long-lasting. Gout nodes in this case are small in size. In severe forms of the disease, large tophi may form, which will destroy the joint and underlying tissues. A person with this form becomes disabled. As a preventive measure, a course of preventive treatment is recommended. It involves taking allopurinol over a long period of time. Frequent walks and exercise are also recommended.
Mostly, already formed gouty nodes have a static position for a long time and do not progress in size. But if gout is not treated and the disease progresses, this can cause the growth of nodes.
They can reach such a size that the tissue underneath them begins to collapse. In this case, very severe pain and inflammation occur in the area of the organ where the tophi is located. Therefore, it is very important to follow all the doctor’s instructions so as not to start the disease.

Gout is a joint disease. The reason for its development is an increased level of uric acid in the body.
The first signs of the disease are manifested by acute attacks of gouty arthritis, which periodically make themselves felt. As the disease progresses, nodules form, known as gout tophi. During attacks of gouty arthritis, the patient's body temperature may rise. As a rule, after certain tests and diagnostics, the doctor prescribes individual treatment, since further actions depend on the cause and stage of the disease.
Acute joint swelling causes severe pain in the patient
As already mentioned, gout first manifests itself as an attack of gouty arthritis. Most often this occurs after heavy consumption of meat or alcohol. Pain in the joint is characterized as pressing, very strong, and mainly occurs at night or in the evening. In most cases, gout affects the joint of the big toe. Less commonly, it can be elbow, knee and other joints.
Unbearable pain periodically occurs in the area of the affected joint, and swelling and redness can often be observed. Mobility of the affected limb can significantly increase pain. The affected joint, first of all, needs to be provided with maximum rest. Next, it is very important to consult a doctor without delay.
After tests show the level of uric acid in the patient’s body, the cause of the disease is identified, and only then treatment is prescribed.
Most often, gout occurs in men. In women, in most cases, this disease occurs after 50 years, when the level of estrogen (female sex hormones) decreases significantly. As is known, estrogens, when they are normal, have a positive effect on metabolism. Consequently, their loss causes metabolic disorders in the body and, accordingly, a tendency to gout.
Gout is often observed in people who abuse alcoholic beverages and in those who are overweight. Among other things, gout is inherited. A person who has a hereditary predisposition to this disease automatically falls into the risk group.
Excess of these foods leads to illness
Due to the fact that gout is a consequence of impaired metabolism, increased levels of uric acid are explained by two main factors: the inability of the kidneys, for any reason, to remove normal amounts of uric acid from the body, or its increased content, which even healthy kidneys are not able to cope with. This can lead to urolithiasis. Therefore, the patient's medical history is somehow related to kidney function.
ICD 10 - international classification of disease No. 10. The ICD 10 code for gout is M10, which refers to the microcrystalline type of arthritis. The cause is impaired renal function.
In turn, according to ICD 10, gout is divided as follows:
The disease disrupts the functioning of internal organs and the kidneys are most affected
So, elevated uric acid levels can often be associated with kidney problems. Concomitant kidney diseases reduce their filtration function, which leads to the gradual deposition of urate crystals throughout the body. As a rule, the joints suffer the most: there is no blood supply there, which allows urate crystals to better gain a foothold in them. In addition, gout can occur as a result of arterial hypertension, and the disease can be triggered by certain groups of medications, the use of which is required in the treatment of many serious diseases.
Inflammation of the affected joint, accompanied by unbearable pain, is characterized as an acute attack of the disease. If the disease is not treated, the process continues and the formation of nodules called tophi occurs. Tophi indicate that the disease has become chronic. Their appearance is explained by an excess amount of uric acid in the body, deposits of which can often be seen in the elbow joints, in the heels, and in the cartilage of the ears. The accumulated salt crystals gradually form nodules. They are white or yellowish in color and can be easily felt through the skin.
Tophi do not cause serious harm to the body. Their appearance only indicates that the patient’s body has a metabolic disorder and gout has entered the chronic stage. Tophi are painless, but when they appear, intensive treatment of the disease is required. With gout, this is especially necessary, since the disease most often develops from acute to chronic form and causes suffering to the patient. The use of medications gradually relieves the acute symptoms of an attack, but in order to ensure that exacerbations do not recur, it is necessary to strictly follow all the doctor’s instructions.
For gout, treatment is divided into two main stages:
MRI shows uric acid buildup in joints
Due to the fact that with gout, the first thing that occurs is an attack of gouty arthritis, which is accompanied by unbearable pain, treatment in most cases is carried out in a hospital setting. At the same time, all the necessary tests are taken, a full diagnosis is carried out, and the causes of the disease are determined. Tests are necessary to determine the nature of the disease (reactive or infectious). Once an accurate diagnosis has been made, re-hospitalization is not required.
When all the necessary tests have been passed, doctors prescribe non-steroidal anti-inflammatory drugs (NSAIDs) to relieve a gout attack. These drugs include ibuprofen, naproxen, etc. Using these drugs, you can not only relieve severe pain, but also stop the inflammatory process in the affected joint. Complex treatment of an acute attack with the use of anti-inflammatory drugs is prescribed by the attending physician, who also determines the necessary dosages.
For the complete treatment of gout, the use of non-steroidal anti-inflammatory drugs is not enough. With their help, you can only relieve the symptoms of an acute gout attack.
The action of the drug colchicine is aimed at reducing the inflammatory process during an acute attack of gout. The use of colchicine effectively relieves pain and gradually restores the function of the affected joint. The use of the drug must be prescribed by the attending physician, since it must be taken according to a special regimen.
If the use of colchicine does not give the desired result, the patient is prescribed hormonal agents (glucocorticoids) that can suppress the inflammatory process in the affected joint. Injections (betamethasone or methylprednisolone) are made into the area of the affected joint. With the help of these drugs, doctors suppress all the symptoms of an acute gout attack in 1-2 days. Next, basic treatment measures for the disease are taken, aimed at restoring normal metabolism in the body.
Correct and timely measures can reduce the frequency of gout attacks. It is not recommended to treat the disease yourself, since, first of all, it is necessary to undergo certain tests and make sure that the kidneys are healthy.
When gout develops, it is very important to monitor the patient’s diet. There are a number of foods that contain high amounts of purines, and such foods should be consumed in moderate portions. By following a diet and taking medications prescribed by your doctor after the first attack, you can prevent further development of the disease. It is immediately worth noting that the list of undesirable products is quite extensive. But, given the consequences of the disease, many patients will probably prefer to follow a diet.

To avoid the development of the disease, it is very important to monitor your diet and not overindulge in unwanted foods. At the first symptoms of gout, you should immediately consult a doctor. Otherwise, the disease may become chronic, and the complications of gout are very dangerous.
