Short-term studies have shown a temporary increase in uric acid levels when starting a strict low-carb (i.e. keto) diet. This effect disappears after about six weeks, and uric acid levels return to their original value, or even lower.
Studies have shown no significant change in uric acid levels when following a low-carbohydrate diet over several months or years.
After dozens of high-quality studies comparing low-carb diets with other diets, there is no data showing an increased risk of gout, although no studies have focused specifically on this aspect.
Doctors who regularly treat patients on low-carb diets do not see an increase in gout attacks, even during the initial period. So, if there is an increase in risk over the first few weeks, it is small or moderate. Long-term uric acid levels usually decrease when following a low-carb diet, as do other markers of metabolic syndrome, and even patients with gout have a chance of getting rid of it.
As a bonus, these lifestyle changes can have many other positive benefits for your weight and health. However, if this is not enough, the medicine allopurinol is very effective in protecting against gout.
