Traditionally, October 12 is celebrated as World Arthritis Day, the main goal of which is to attract the attention of the medical community and the population to the problems of rheumatic diseases, highlighting the importance of early diagnosis, treatment, prevention and rehabilitation.
The event, held on October 12, 2017 in the Republic of Belarus, raises one of the significant problems of modern rheumatology - the problem of early diagnosis of psoriatic arthritis. The relevance of this problem is due to the frequent damage to people of young working age, the rapid disability of patients due to untimely treatment and non-compliance with the principles of treatment.
Psoriatic arthritis is a form of arthritis that affects people with psoriasis. It is considered the second most common inflammatory joint disease after rheumatoid arthritis. It is diagnosed in 7-39% of patients with psoriasis. Psoriasis is a skin disease in which plaques form on the skin, consisting of dense areas covered with dry, keratinized layers of skin cells. The disease belongs to the group of autoimmune diseases, that is, it occurs when the immune system, which normally protects the body from harmful agents, such as bacteria and viruses, mistakenly begins to attack healthy cells and tissues. An abnormal immune response causes joint inflammation and overproduction of skin cells. Heredity and environment are thought to play a role. Many people with psoriatic arthritis have close relatives (parents, siblings) with psoriasis. Researchers have discovered specific genetic markers associated with this disease. If one of your close relatives suffers from psoriatic arthritis, this does not mean that you will also develop the disease, it only means that you have a predisposition to this disease. Physical trauma or an infectious (viral) disease can trigger the development of psoriatic arthritis in people with a hereditary predisposition. Many people first develop psoriasis and later develop joint inflammation - arthritis. But sometimes arthritis precedes psoriasis.
Joint pain, stiffness and swelling are among the main symptoms of psoriatic arthritis. Any joints can be affected, including the small joints of the hands, as well as the spine. The development of the disease is unpredictable: from moderate manifestations to pronounced changes in the joints. Both psoriasis and psoriatic arthritis are chronic diseases; over the years they tend to progress and become protracted, but there may be periods of “calm” (remission), alternating with periods of exacerbation.
Joint damage in psoriatic arthritis is often asymmetrical. When arthritis affects the hands and feet, inflammation and swelling of the tendons can cause the fingers to become characteristically thick (dactylitis) and resemble small sausages (“sausage toes”). The small joints of the hands and feet near the nails (distal interphalangeal joints) may be involved. Nails also quite often become characteristic of psoriasis: they thicken, change color, and pits appear in them. A form of psoriatic arthritis called spondylitis is associated with inflammation of the spine. Characterized by back pain, stiffness in the cervical and lumbar regions, as well as inflammation of the joints between the spine and pelvis (sacroiliac joints). A small percentage of people with psoriatic arthritis have its severe (destructive) form - arthritis mutilans. Over the years, this form of arthritis destroys the small bones of the hands, leading to permanent deformity and loss of function.

Psoriatic arthritis can begin suddenly or gradually, but either way, the disease can seriously damage your joints if left untreated. There is no specific test to confirm psoriatic arthritis, so the doctor will evaluate all symptoms and prescribe tests to rule out other possible joint diseases. A timely diagnosis and timely treatment will slow down the progression of the disease and avoid irreversible consequences.

October 12 is celebrated as Arthritis Day in many countries around the world. This year the Russian Federation celebrated this day for the first time. On October 10, the Scientific Center for Children's Health of the Russian Academy of Medical Sciences, with the support of the Interregional public organization "Renaissance" (assisting disabled children suffering from rheumatic diseases ), held a children's holiday, the motto of which was "Think positively!" According to official statistics, more than 50,000 patients with rheumatic diseases are registered in the Russian Federation. The purpose of the holiday was to attract the attention of the public, health authorities and authorities to their problems.
Academician A.A. Baranov, during a meeting with journalists, noted that not only in pediatric rheumatology , but in the entire field of healthcare, there are many problems, some of which have already been solved. But there are those that still need to be resolved, including at the state level (we are talking about improving legislation). This will require titanic efforts from the union of pediatricians, as well as parents of young patients.
“Currently, little attention is paid to the problems of pediatric rheumatology by health authorities, the public and the media. They receive virtually no press coverage, despite their seriousness. Disability in rheumatic diseases in children, especially rheumatoid arthritis, develops in the first 10 years of the disease . In addition, rheumatoid arthritis, as the most common and most severe rheumatic disease in children, negatively affects the growth of the child's body . Noise control methods are regulated by GOST 12.1.029-80 [6] and SNiP 23-03-2003 [11].
Noise, depending on the source, is divided into mechanical (vibration), aerodynamic , electromagnetic , hydrodynamic . When developing means that reduce noise along the path of its propagation, it is necessary to take into account the features of these paths, namely, to distinguish airborne noise (transmitted through the air) and structural noise (propagating through the elements of the building’s building structures).
To reduce noise in industrial premises, various methods are used:
reducing the noise level at its source;
noise reduction on transmission paths (sound absorption, sound insulation, installation of noise mufflers);
rational placement of equipment;
use of personal protective equipment.
The classification of collective noise protection means is presented in Fig. 2.

The most effective way to combat noise is at its source . The noise of mechanisms arises due to elastic vibrations of both the entire mechanism and its individual parts. To reduce mechanical noise, it is necessary to timely repair equipment, replace impact processes with non-impact ones, apply forced lubrication of rubbing surfaces, apply balancing of rotating parts, increase the accuracy class of machining parts and gears; replace reciprocating movements with rotational ones, stamping by pressing, riveting by welding, cutting off, spur gears with helical and chevron gears (allows you to reduce the sound level to 5 dBA).
A significant reduction in noise is achieved by replacing rolling bearings with plain bearings (noise is reduced by 10–15 dBA), gear and chain drives with V-belt or toothed-belt drives (noise is reduced by 10–15 dBA), and metal parts with plastic parts.
Reducing aerodynamic noise can be achieved by reducing the gas flow velocity, improving the aerodynamics of the structure, sound insulation and installing mufflers. Electromagnetic noise is reduced by design changes in electrical machines.
Architectural and planning methods of noise protection include :
rational acoustic solutions for building layouts and master plans of facilities (concentration of workshops with high noise levels and their removal from other production premises);
rational placement of technological equipment and workplaces;
rational acoustic planning of areas and operating modes of noisy equipment, vehicle movement and traffic flows;
creation of noise protection zones in various places where people are;
forest plantations (reduce noise by 10 - 15 dB).
The intensity of noise in rooms depends not only on direct, but also on reflected sound, which can be reduced by increasing the sound absorption area of the room, i.e. must be used:
covering the internal surfaces of the room with sound-absorbing linings. This acoustic treatment of rooms allows you to reduce the sound level to 45 dBA;
placement of piece sound absorbers in rooms (volumetric bodies filled with sound-absorbing material and suspended from the ceiling).
Organizational and technical methods of noise protection include :
the use of low-noise technological processes by changing production technology, processing methods, and transportation of material;
equipping noisy machines with remote controls (reduces noise by 20–30 dBA);
the use of low-noise machines, changes in the structural elements of machines, their assembly units;
installation of screens (coated with soundproofing materials) between the machine and the workplace;
improvement of technology for repair and maintenance of machines;
the use of rational work and rest schedules for workers in noisy enterprises;
reducing time spent in noise conditions;
hygienic control of noise levels in workplaces.
Organizational and technical methods ensure a reduction in sound level by 5–10 dBA.
In cases where the listed methods do not provide the necessary noise reduction, personal protective equipment ( PPE ) is used - noise suppressors.
Noise suppressors according to GOST 12.4.011–89 [8] are divided into three types:
headphones (Fig. 3) covering the auricle. Depending on the frequency, they provide a noise reduction of 7–47 dB. Headphones provide protection most effectively at high frequencies;
inserts (Fig. 3) covering the external auditory canal (earplugs). Depending on the frequency, they provide a noise reduction of 5–20 dB. They are made from special ultra-thin fiber, as well as rubber or ebonite;
helmets covering part of the head and auricle. Used at very high noise levels (more than 120 dB).

The effectiveness of personal protective equipment can be ensured by their correct selection depending on the levels and spectrum of noise.
In enterprises, sound zones above 80 dBA must be marked with safety signs, and those working in these zones must be provided with personal protective equipment. Even short-term stay in areas with sound pressure exceeding 135 dBA in any frequency band is prohibited.
The technical specifications for machines and passports must indicate the values of the noise characteristics of machines; noise measurement is carried out in accordance with GOST 12.1.050-86 [7].
It is important to reduce the dangerous effects of noise on the human body from periodic medical examinations of health status and treatment and preventive measures.

On October 12, many countries celebrate World Arthritis Day, which is held annually on the initiative of the World Health Organization (WHO) since 1996 (in Russia - since 2006).
The main goals of this Day are to attract the attention of the general public around the world to the problem of rheumatological diseases, increase awareness of arthritis in all its forms, unite the efforts of doctors, patients and the public to address issues of prevention, early diagnosis, medical and social rehabilitation of patients with this disease.
Arthritis is a serious inflammatory disease of the soft tissues of a joint or several joints, which, if left untreated, leads not only to pain and limitation of movement, but also to disability and loss of ability to work. The causes of the disease are different - arthritis often occurs due to repeated minor injuries, open or closed joint injuries, frequent physical strain, or a genetic predisposition. In addition, the development of arthritis is provoked by viral and bacterial infections, hypothermia, physical and mental overload, and even excessive smoking. In some cases, the factors of occurrence and mechanisms of development of the disease are not fully understood.
The inflammatory process causes disruption of the structure and function of the joints, which leads to pain, stiffness, swelling, which ultimately leads to the development of arthrosis (irreversible destruction of the joint) and disability of the patient.
In general, arthritis refers to a large group of joint diseases, which includes more than 100 diagnoses, the most common of which are arthrosis and rheumatoid arthritis. For example, in Russia alone, according to the Ministry of Health, there are more than 10 million patients with arthrosis and about 300 thousand Russians suffer from rheumatoid arthritis (about a third of them have severe forms of disability).
The most dangerous thing is that anyone can be at risk of developing this disease - from a baby to an old man. But most often, severe rheumatic joint diseases associated with the rapid development of arthritis affect the working population of the planet - between the ages of 30 and 50 years. By the way, women suffer from this disease 5 times more often than men. Moreover, joint diseases cannot always be recognized immediately because of their “hiddenness” and “camouflage”, and subsequently they require long-term and constant medical observation and treatment, and patients require rehabilitation and social adaptation.
Therefore, underestimating the importance of the problem associated with pain in the joints and spine leads to late referral of patients to doctors and, as a result, reduces the patients’ chances of recovery or achieving remission, and increases the risk of developing disability.
Today, joint diseases have literally become a global problem. According to WHO, every tenth person on the planet suffers from arthritis. And over the last decade, the trend towards an increase in the number of diseases of the musculoskeletal system and connective tissue, including arthritis, has not decreased, and rheumatological diseases occupy high positions in the ranking of causes of morbidity and disability.
Therefore, it is important to remember that only early diagnosis and timely treatment can prevent serious consequences for a patient with arthritis. According to doctors, with early diagnosis and individually selected treatment, arthritis can be cured. Therefore, diagnosing arthritis is a very important step towards recovery, and modern methods of therapy can significantly improve the quality of life of patients.
And although the causes of arthritis are varied, doctors are unanimous in their opinion that the best cure for arthritis is prevention. The tick has 4 stages of development - egg, larval, 2 in the form of a nymph. Adult spider mites appear as reddish and brownish spots on plants. An excellent effect is obtained by using such a product as a drug intended for removing fleas from animals. It should be mixed with water, then sprayed on the eggplants. Among the popularly known methods, it is worth mentioning the use of tobacco stems and leaves. Consumption is about 45 grams of dry material per 1 liter of water. Before starting to treat the plantings, the product must be diluted with water. This is done at a ratio of 1:2. The use of hot pepper fruits against parasites has been popular for many years. Making a solution based on it is quite simple. You need to take 100 grams of fruit per 1 liter of liquid.
There are several proven recipes for getting rid of larvae.
You can take 120 grams of potato tops per 1 liter of water or 270 grams of garlic bulbs per the same amount of liquid. A combination of 400 grams of crushed tomato parts, which are placed in 1 liter of water, also works well. Don't forget to dilute this infusion before use. Dandelion roots and foliage have proven themselves to be excellent. You should take about 30 grams and place in 1 liter of water. You can use yarrow herb. In this case, 80 grams of dry raw materials are poured with a liter of liquid. Against larvae, a soap-alcohol solution is often used to treat the surface of foliage and stems. It is recommended to add a little washing powder or soap to the decoctions. Then the homemade product will stick to the crops normally.
There are many highly effective drugs on the market today. Among them are such insectoacaricides as “Fitoferm” and “Aktellik”. The first is slightly toxic to humans. It is recommended to carry out processing indoors. Afterwards, wash your hands and face thoroughly with soap. The frequency of treatment is 3 times a week. The second drug should be used on open areas. Among acaricidal agents, “Sunmite” and “Neoron” can be noted. The first one is good at destroying both adult pests and eggs. The second makes it possible to eliminate mites on eggplants after just 2 treatments.
There are also biological preparations that do not have a harmful effect on the human body and his pets. These are the so-called phytoseiulus and amblyseiulus - predatory mites that feed on arachnids. Every day they can destroy about 10 parasite eggs. Sometimes a method is used to disinfect a greenhouse by burning sulfur in the form of briquettes. In case of excessive growth of the parasite population, it is recommended to spray the crops with colloidal sulfur. This is done at the rate of 70 grams of product per 1 bucket of water.
Prevention is always better than control. Following certain rules will help prevent eggplants from becoming infested with spider mites.
Crops need to be sprayed from the outside from time to time. Since parasites of this type most often appear due to insufficient air humidity. It is necessary to ensure that moisture does not stagnate in the middle of the plant. Otherwise, rotting cannot be avoided.
