Arches of the foot. The bones of the metatarsus and tarsus do not lie in the same plane, but form longitudinal arches, convexly facing upward. The arches of the foot provide a shock-absorbing function during static loads and walking, and also prevent compression of soft tissues during movement and create favorable conditions for normal blood circulation.
Flat feet is a flattening of the arches of the foot - transverse, longitudinal, or both. There are the following reasons for the development of flat feet:
– congenital – due to improper development of the foot skeleton;
– rachitic – due to softening of the bones of the foot due to a lack of vitamin D in the body;
– paralytic – due to paralysis of the muscles that support the arch of the foot (for example, with polio);
– traumatic – due to a violation of the normal configuration of the bones of the foot that form the arch;

Excess body weight leads to the fact that the foot simply cannot cope with this weight. This circumstance is especially affected at certain age stages of active human growth - at 6 - 8 and at 11 - 14 years. Another period of formation of excess body weight (and especially rapid) is a woman’s pregnancy. If we consider that at the same time the natural process of leaching calcium from a woman’s bones occurs, then the frequent cases of flattening of the arches of the feet at this time become understandable, and flat feet persist at all subsequent stages of life.
Incorrectly selected shoes . The size, style and rigidity of the bottom of the shoe should not impede the development of the foot. A hard sole makes it difficult to walk (walks “like on stilts”), reduces the performance of the lower leg muscles, increases the temperature of the skin of the legs and sweating. A smaller shoe size causes discomfort and the appearance of calluses, interferes with the blood circulation of the feet, which is why they freeze in winter; with a large size, the foot in shoes seems to “blur”, which can lead to flat feet.
Special mention should be made about high-heeled shoes. The fact is that with a heel higher than 2–3 cm, the center of gravity of the body shifts forward, and the bulk of the body weight falls on the forefoot (Table 8), which leads to the development of flat feet and is a serious prerequisite for the appearance of not only various types of deformities of the foot and fingers, but also varicose veins of the lower extremities. In addition, due to the shift in the center of gravity, in order to maintain balance, the girl moves her pelvis forward, which causes it to become narrow (it is clear that in this case, the subsequent development of the fetus in it is difficult), and the girl also moves her shoulders forward - so stooping develops. A shift in the center of gravity along with an unsteady gait leads to the formation of incorrect posture. This is especially dangerous for people under 20–25 years of age, when the formation of the skeleton has not yet completed.
But shoes without any heels are also not recommended, so the best option is shoes with a heel height of 2–3 cm - then the body weight is evenly distributed over the entire foot, and walking will not be tiring.

The main objective criterion for diagnosing flatfoot is an increase in the Chizhin index, determined by the plantography method.
There are three degrees of flat feet:
I – valgus position of the heel with slight flattening of the transverse arch of the foot;
II – valgus position of the heel and flattening of the transverse arch of the foot;
III – heel valgus, flat transverse arch and forefoot abduction.
Prevention of flat feet includes training the calf, anterior and posterior tibialis muscles, long extensor fingers, which supinate the hindfoot and rotate the lower leg outward, and small muscles of the foot that strengthen the longitudinal arch. The main focus in correcting foot deformities is training the muscles that form the arches of the feet. First of all, these are the gastrocnemius, anterior and posterior tibialis muscles, flexors of the fingers and thumb, muscles that pronate the foot, etc.

During the introductory period, exercises are carried out from lightweight starting positions - sitting and lying down. To increase the total load, dynamic exercises are alternated with relaxation of the muscles of the lower extremities. It is necessary to achieve equalization of the tone of the muscles that hold the correct position of the foot. This can be done by walking on tiptoes, on the outer edge of the foot, grasping small objects with the toes in a sitting position, etc. A mandatory requirement, as with spinal deformities, is to achieve a state of fatigue of the muscles that determine the state of the arch of the foot. The most important condition for achieving the correction effect is to perform the exercises multiple times during the day in such a way that each successive execution has an aftereffect after the previous one. In the complex of functional therapy for flat feet, massage should also occupy an important place, which helps relieve hypertonicity of some muscles and increase the tone of weakened ones. But it should be noted that the isolated use of massage without appropriate functional support with the help of exercise therapy cannot give a positive result in the correction of flat feet.
In the main period , attention is paid to further increasing the load, for which they use exercises with resistance, with objects, jumps, various types of walking (on toes, heels 14, outer edge of the feet), rolling from toes to heels and back, in the initial position of the front support part of the foot on support, lifting on toes, etc.

Swimming is extremely effective in the treatment of flat feet in the exercise therapy system. When swimming, a large load is placed on the muscles of the foot, due to which the muscles that hold the arches of the foot, as well as the muscles of the lower leg, are strengthened and developed. In addition, there is no static effect on the foot, so swimming can be recommended as a means of preventing and treating flat feet. Schoolchildren with flat feet (with some exceptions) belong to the preparatory medical group, i.e. are engaged in the main group, but with some contraindications to the use of certain exercises. In addition, such children should receive individual assignments so that the number of daily lessons, especially in the introductory and main periods, is at least three.
Exercise therapy in the treatment of flat feet should be combined with other measures: hygienic, selection of rational shoes, orthopedic devices, determination of the optimal motor mode, etc.
Favorable treatment results are expressed in the reduction or disappearance of discomfort and pain in the legs during prolonged exercise, and in the normalization of the position of the feet and gait. But even if objective studies (Chizhin index) do not show a decrease in flat feet, with systematic training of the articular-ligamentous apparatus in a person, many unfavorable subjective sensations (in particular, headaches) smooth out and disappear and overall performance increases. Moreover, practice shows that in this case, even the presence of an objective diagnosis of flat feet does not prevent a person from playing sports and achieving high results in many types of sports.

According to etiology, there are five types of flat foot: congenital, rachitic, paralytic, traumatic, static. The most common is static flatfoot. Acquired flat feet can be caused by paralysis and paresis of the muscles of the lower extremities, complications after traumatic injuries, impaired calcium-phosphorus metabolism, and rickets.
With flat feet, there is a lack of shock-absorbing properties of the foot, which causes rapid fatigue when walking, pain in the feet, hips, lower legs, and lumbar region.
The development of flat feet in humans often occurs due to an increase in body weight, and in athletes - as a result of the systematic use of excessive loads. The cause of flat feet is also wearing shoes that do not correspond to working and living conditions. In the process of walking, the human body is subjected to various shocks and shocks, which are largely absorbed by the foot.
The shock-absorbing function of the foot is provided by the longitudinal and transverse arches, thanks to the interaction of the skeleton, joints and muscles of the foot, forming a complex elastic system. Transverse flatfoot in combination with other deformities is 55.2%, longitudinal flatfoot in combination with other deformities of the feet is 29.3%, outward deviation of the first toe in combination with other deformities is 13.2%, hammertoes are 9.9%.
In the case of progression of longitudinal flatfoot, the length of the feet increases mainly due to the lowering of the longitudinal arch, and with the development of transverse flatfoot, the length of the feet decreases due to the fan-shaped divergence of the metatarsal bones, the outward deviation of the first toe and the hammer-shaped deformation of the middle toe.
A flat foot can be congenital, or it can develop during a person’s life. With flat feet, the normal function of both the foot itself and the entire musculoskeletal system is disrupted.
