It is believed that venous diseases have no significance for blood circulation in general. Veins, in particular the so-called varicose veins, their inflammation, thrombosis are considered purely local diseases. This, at least in relation to varicose veins and phlebectasia in general, is apparently not entirely correct. Varicose veins - for example, hemorrhoids, of course, can be the result of purely local causes that impede the outflow of venous blood from the area. But there is no doubt that very often a constitutional genotypic factor plays a role in the origin of these varicose veins. This circumstance forces us to consider varicose veins as an indicator of the inferiority of the veins in a given person and gives the right to assume a general decrease in the tone of their muscles and, due to this, a decrease in their main function - the function of supplying blood to the heart. This function of the veins is carried out, to a large extent, by the active tone of their muscles, changing the lumen of the veins as necessary to deliver more or less blood to the heart from the periphery. Apparently, veins are mainly the part of the vascular system in which blood is retained during reduced circulation and which plays the role of a blood depot. This function of the veins as a depot and their function of supplying blood to the heart are closely related to each other. Indeed, people who have varicose veins, mainly in the lower extremities, are often characterized by decreased performance, weakness, a tendency to faint and a general tendency to develop circulatory failure.
B. VEIN THROMBOSIS AND THROMBOPHLEBITIS
A distinction is usually made between venous thrombosis and inflammation of the veins - phlebitis.
Vein thrombosis is said to exist if there are no clear signs of inflammation of the vein wall. Phlebitis is usually accompanied by thrombosis; therefore, the term “phlebitis” is often replaced with the term “thrombophlebitis”. The main cause of all vein thrombosis is a change in the wall of the veins, in particular its endothelial cover.
Most often, these changes are caused by an inflammatory bacterial process in the vein wall. This inflammation most often spreads to the vein from surrounding tissues when the corresponding organ is diseased - this is mainly the pathogenesis of the above-mentioned thrombophlebitis of internal organs, thrombophlebitis in the area of infected wounds, etc. In other cases, the inflammatory process may be the result of a hematogenous infection of the walls of the vein - this is what you need present the pathogenesis of thrombophlebitis, which so often accompanies various common infectious diseases (abdominal, relapsing, typhus, scarlet fever, sepsis, pneumonia, septic endocarditis, etc.).
However, often with vein thrombosis it is not possible to determine the presence of an inflammatory process in the wall of the veins and, in particular, the localization of a bacterial infectious process in it. But undoubtedly, in these cases, thrombosis is primarily the result of changes in the endothelial cover of the vein. These changes are considered as a result of changes in the reactivity, the allergic state of the endothelium lining the veins. This explains, perhaps, such frequent changes in it and, at the same time, such frequent development of vein thrombosis in various acute and subacute general infectious diseases. One can imagine that this change in the reactivity of the endothelium is associated with changes in its physicochemical properties that contribute to the formation of blood clots. In an allergic or hyperergic state of the venous endothelium, it is possible that the cause of thrombosis may be changes in the vein wall as a result of intoxication and as a result of malnutrition (for example, so-called arrowroot thrombosis).
If vein thrombosis occurs more often than arterial thrombosis, this is, of course, explained by the much slower blood flow in the veins. This is an illustration of the importance that slowing down blood flow generally plays in the pathogenesis of thrombosis. This additional factor also explains the predominant localization of thrombosis in those areas and in those veins where blood flow is slowed down - primarily in varicose veins and in the veins of the lower extremities in general - and, perhaps, to an even greater extent in the veins of the portal system.
The slowdown in blood flow also explains the fact that in all diseases of the circulatory system associated with a slowdown in blood flow, the tendency to form blood clots due to associated infectious processes is especially high. But undoubtedly, a slowdown in blood flow in itself cannot cause thrombosis - a change in the endothelium lining the vein is always necessary. The same applies to increased blood clotting: it in itself does not lead to thrombosis, but in the presence of changes in the endothelium or a slowdown in blood flow, it can contribute to its development.
Normally, the endothelial layer is not wetted by blood. The first change that contributes to the deposition of a blood clot is the appearance of a certain stickiness of the endothelium. It allows blood platelets to settle on its surface; then, through agglutination, accumulation of these elements occurs, and later fibrin is released. This is the process of formation of a blood clot in the place where it originated, where its primary localization is. This main blood clot is joined by the formation of a blood clot above and below its location - the blood clot often grows over a very long distance. This growth of a blood clot occurs through normal blood clotting, and not through the formation of a blood clot primarily from blood platelets.
Undoubtedly, over the past two to three decades, vein thrombosis and thrombophlebitis have become increasingly common. It has not yet been possible to find a completely satisfactory explanation for this phenomenon. In any case, it cannot be associated, as was supposed, with the significant recent spread of the intravenous method of using drugs.
The most dangerous, often fatal consequence of venous thrombosis is pulmonary embolism. The more acute the phlebitis, the greater the tendency to disintegrate the blood clot, the greater the tendency to embolism. Typically, blood clots in the veins undergo, as they say, organization, i.e. they grow with connective tissue and turn into a connective tissue cord. Less commonly, some restoration of the lumen of the vessel occurs through the formation of new vessels in the connective tissue that fills the lumen of the vein. These vessels, connecting with each other, lead to “recanalization” of the vein.
The clinical picture of vein thrombosis and thrombophlebitis of the extremities is different depending on the underlying disease on the basis of which this always “second” disease develops. The most characteristic picture is given by thrombophlebitis, which develops during an infectious disease or, more often, during the first period of recovery from it, for example, from typhoid fever. Usually the first manifestation is pain that suddenly appears in the limb. Its localization is varied; if thrombophlebitis develops, as happens in the vast majority of cases, in the lower limb, then the pain is felt either in the calf, then in the sole, or in the thigh. Often at this time, if thrombosis begins in small veins, an ordinary study will not reveal anything yet. If the process immediately involves a large vein located relatively superficially, for example vena saphena, then it is palpable in the form of a painful cord: after 1-2-3 days swelling of the limb appears. Along with the onset of pain, an increase in temperature appears, which is usually not particularly high and is of a remitting irregular nature.
At the same time, moderate neutrophilic leukocytosis develops and ROE accelerates. Painful phenomena continue, they can be quite intense, but never reach the same degree as when the lumen of the artery is closed. Only sometimes, at the beginning of the development of the disease, the picture resembles an arterial embolism both in the intensity of pain and in the coldness of the limb; in these cases, one must think about a temporary, reflex spastic contraction of the arteries of a given limb. Subsequently, swelling, pallor and soreness of the limbs (phlegmasia alba dolens of old clinicians) dominate; the area of the corresponding vein trunk (femoral or popliteal vein) is especially painful; sometimes it can be felt in the form of a cord. Thrombosis often spreads upward (along the blood stream) and then reaches the inferior vena cava. In these cases, thrombosis usually spreads to the other leg. In such cases, the risk of pulmonary artery embolization is especially high.
With such extensive thrombosis, the course of the disease is long. The fever continues for weeks. It, together with leukocytosis and ROE, is important as an indicator of the ongoing activity of the inflammatory process in the vein wall. If the constant swelling eventually disappears and the temperature becomes normal, then often for a long time, and often forever, there remains a tendency to edema of a given limb as a manifestation of insufficiency of the venous outflow tract due to the closure of the lumen of the main venous trunk. Swelling appears with general fatigue, after a long stay in a standing position, etc. The collateral path through the saphenous veins is often manifested by varicose veins.
Thrombophlebitis usually occurs more easily with varicose veins of the lower extremities. Dilated veins, when they are inflamed and their lumen is closed by a thrombus, are palpated in the form of hard, painful cords. The surrounding tissues are swollen, infiltrated with inflammatory exudate and colored brown-red or cyanotic due to impregnation with altered blood pigment. There is usually swelling of the limb. The process is often protracted, passes from one vein to another, and usually recurs. The temperature during an exacerbation rises moderately.
The third option is septic thrombophlebitis, usually spreading to the veins of the lower extremities from the pelvic veins with septic postpartum diseases of the uterus. In these cases, a high remitting temperature is observed, accompanied by chills and sweats. Pulmonary embolism is a very common complication of this form.
Finally, in some cases, vein thrombosis occurs completely latently, and its first appearance is the ominous picture of pulmonary embolism (see the section on pulmonary diseases).
The prognosis for thrombophlebitis is always clouded by the possibility of pulmonary embolism. When the trunk is closed by its embolus, death occurs within minutes with symptoms of severe suffocation. With septic thrombophlebitis, usually localized in the pelvic veins (and often complicating septic postpartum metritis or infected thrombosis of varicose hemorrhoidal veins), embolism of the branches of the pulmonary artery leads to the development of pulmonary abscesses or pulmonary gangrene.
Thrombophlebitis has a high tendency to recur. This is understandable, since they lead to a slowdown in blood flow in the veins of a given area, to their expansion and changes in their walls. All this in turn contributes to thrombosis.
But still, very often thrombophlebitis ends in complete recovery, usually after a long period of bed rest, lasting weeks or even months.
Vein thrombosis or thrombophlebitis is usually a complication or consequence of another disease. Therefore, the prevention of thrombophlebitis, on the one hand, coincides to a large extent with the prevention and treatment of these diseases, and on the other hand, it consists in preventing the development of thrombophlebitis in them. The latter can be achieved to a certain extent by rational nutrition, treatment and care, which helps improve the general condition of patients and enhance the functions of the cardiovascular system.
Recently it was advised to prevent the development of postoperative and postpartum thrombosis by an earlier start of passive and active movements, possibly by getting up early in patients. After surgery and childbirth, this technique was meant to eliminate one of the factors contributing to thrombosis - slowing down blood flow. But since this factor is only of secondary importance, this technique does not have significant success.
With thrombophlebitis, the patient should keep complete rest in bed; urination and defecation should be done in a supine position. Re-making the bed should be done with the utmost care. The diseased limb is wrapped in cotton wool and placed motionless in a splint or pillows in a slightly elevated position to facilitate the outflow of blood. All physiotherapeutic procedures are strictly contraindicated. Thrombophlebitis itself does not require drug treatment. It is necessary to take care of circulatory function and good nutrition.
The period of immobility of the patient and the diseased limb is determined by the duration of manifestations of the activity of the inflammatory process. Only after the swelling has disappeared and the thrombosed vein has again become painless and 10-15 days after the temperature has become normal, can passive movements of the affected limb begin. The patient can get up no earlier than half a month after all manifestations of the disease disappear.
The first thing to do in case of thrombophlebitis is to seek help from a qualified phlebologist surgeon. Massage is strictly contraindicated for thrombophlebitis of the deep veins of the lower extremities. Any manipulation of the legs can cause a blood clot to break off and migrate to the internal organs. The consequences of massage can be unpredictable: from blood poisoning to pulmonary embolism.
Light self-massage is necessary to prevent blood clots when the veins already have varicose areas, but there are no blood clots in them yet. Exercises after surgery to remove thrombophlebitis will not harm. A simple massage will help normalize blood circulation and increase muscle tone in the legs.

The patient can perform simple movements even while lying in bed. It is not worth fixing the affected limb in one position.
