Every doctor has to deal with gout in one form or another. Gout is such a common occurrence here in England that quite a few people are in the habit of treating attacks with the help of the family doctor's favorite colchicum, together with liver remedies and alkaline preparations of lithium, potassium and sodium. Many of us stick to our favorite diets. As for alcohol, one sharp-tongued lady from the Northern District, having heard me recommend Scotch whiskey, well diluted with water, to a patient, said:
Yes, I see you are a lover of scotch tape! It seems to me that the only difference between doctors who treat gout is the choice of whiskey. You recommend scotch, but Dr. Moore loved Irish. I often told daddy that if I were in his place, I would mix scotch tape with Irish, and then I definitely wouldn’t make a mistake in choosing the right product!
The whole cynicism of the situation was that I had a local reputation as a staunch opponent of alcohol in any form, especially in the treatment of gout. And a partial change in my views occurred as follows.
I was treating a village squire for inflammation of the knee joint when his 80-year-old father-in-law, Sir Edward, decided to visit him. When the latter suffered an acute attack of gout, I was invited to treat him too, but my Aconitum and Bryonia did not help. A colleague could not explain which diet is best for gout, that is, when you need port wine, and when you need oranges and water.
But, as I gained experience, I came to some conclusions.
Therefore, I believe that food and stimulants per se are not necessarily the cause of gout attacks. This is the case, I believe. If the fireplace is smoking in the room, then any fuel will fill the room with smoke and soot. It’s the same with gout - any food will cause an attack. The real reason is not the fuel, but the fireplace; not in food, but in the body.
I call such cases of gout asthenic gout , and here stimulants are really needed.
I have reason to believe that Sir James Paget [1814 - 1899, British surgeon and pathologist, along with Rudolf Virchow, one of the founders of scientific pathology. I once recommended to a gentleman who came to me about asthenic gout, to take two glasses of port after dinner. There are cases of gout that have both qualities, asthenic and sthenic, and here the issue of stimulants is very difficult to resolve. In general, I have achieved the best treatment results by recommending changing stimulants rather than sticking to just one. So I'm more of a fan of prescribing a little dry champagne with dinner 1-2 times a week, a little Bordeaux 1-2 times a week, and a little whiskey and soda the other days. But permission must always be taken into account, taking into account heredity, habits and lifestyle. We must always remember that we are dealing with adults, not children. Our job is not to dictate to an ideal subject an ideal diet under ideal conditions. We work with gout patients, and our job is to improve their health, strengthen them and keep them in this condition for as long as possible. In other words, the child needs a completely different regimen and diet than what will be best for an adult with gout.
Nor should the sufferer of gout, man or woman, be limited to the simplest food, saying that if one can live on bread, fruit and water, nothing else should be taken.
In the simplest cases, we can consider asthenic and sthenic forms of gout as congenital and acquired, respectively. Stenic gout is caused by excessive intake or insufficient excretion: a lot is taken in with food, little is excreted with secretions, hence the accumulation of urate soot and soot, products of gout. He received from me Urtica ∅, 12 drops in water twice a day, as I believed that the boils were due to malaria suffered many years ago. He still took quinine for fear that the attacks would return, since any cold could trigger them. After taking Urtica, a severe fever began, so strong that the patient's friends were seriously concerned about his condition. However, he then quickly and completely recovered.
Another patient was also an officer sent home from India. He suffered from "liver" and chronic malaria and received from me 10 drops of Urtica urens twice a day. This, too, was followed by a severe fever with unusually long stages, from which he recovered with Natrum muriaticum 6 every two hours. He then reported that "these powders cured the fever, but great weakness remained."
It is very interesting to note the significant healing effect of Nat. mur. and Urtica for gout, as well as malaria, including its consequences.
When treating gout, the patient is prescribed non-steroidal anti-inflammatory drugs, and the most effective pharmaceutical drug in this group is Arcoxia. This disease is characterized by damage to joints and tissues and is accompanied by severe painful symptoms. The pharmaceutical drug "Arkoksia" is designed to eliminate pain, eliminate inflammation and reduce elevated body temperature. The dosage and duration of gout therapy with the medication in question is determined by the attending physician personally for each patient.
The pharmaceutical product Arcoxia is sold in the form of tablets containing the active ingredient etoricoxib. This substance has a strong anti-inflammatory and analgesic effect. Its principle of action is the ability to inhibit the activity of COX-2 enzymes, as a result of which the metabolism of omega-6-unsaturated fatty acid is disrupted and the level of prostaglandins in the tissues of the nervous system and affected tissue structures is reduced. The drug "Arcoxia" suppresses the pain impulse, reduces joint stiffness, eliminates the source of inflammation and improves the mobility of the musculoskeletal system. The therapeutic effect when using Arcoxia is observed within a quarter of an hour after taking an oral dose of the medicine.

Despite its high effectiveness, not everyone is allowed to use the pharmaceutical drug Arcoxia to cure gout.
